Causes of Inadequate Response to Antihypertensive Drugs

چکیده
Diuretic therapy, by producing a negative salt and water balance, eliminates the false tolerance to sympatholytic drugs that often occurs during long-term monotherapy. This tolerance results from salt and water retention produced by the drugs. Review of published results suggests a primacy for arterial pressure reduction in this fluid because suppressed renal sympathetic activity should facilitate salt and water excretion through lessened a-adrenergic influence on tubular reabsorption, and /3-adrenergic inhibition would diminish renin release thus promoting natriuresis. The return of hypertension that characterizes the false tolerances seems paradoxical because these drugs cause venodilation, which should provide ample storage of expanded blood volume without affecting cardiac output. However, animal studies have suggested that dilated veins have decreased compliance; if that is so, in humans it would mean that fluid retention would be accompanied by a redistribution of blood into the central circulation, with a rise in cardiac output. Knowledge of pharmacokinetic-pharmacodynamic (PK/PD) properties of antihypertensive drugs may optimize drug therapy of hypertension. PK/PD modeling in clinical research could contribute in drug development and clinical practice in several aspects, including an evaluation of the efficacy and safety of antihypertensive agents, enhancement of information during the development process, identificat...
متن کاملCommon Secondary Causes of Resistant Hypertension and Rational for Treatment
Resistant hypertension is defined as uncontrolled blood pressure despite the use of three antihypertensive drugs, including a diuretic, in optimal doses. Treatment resistance can be attributed to poor adherence to antihypertensive drugs, excessive salt intake, physician inertia, inappropriate or inadequate medication, and secondary hypertension. Drug-induced hypertension, obstructive sleep apno...
متن کاملTHE EFFECT OF ANTIHYPERTENSIVE DRUGS IN THE SUPPRESSION OF ATHEROSCLEROSIS
The present investigation was undertaken to evaluate the effect of antihypertensive drugs on aortic atherogenesis in hypercholesterolemic rabbits. In enalapril and nifedipine treated rabbits aortic atherosclerotic plaque involvement was significantly decreased (p<0. Oland p<O. 05, respectively) as compared to the control group. However, in hydrochlorothiazide treated rabbits aortic atheroge...
متن کاملAdverse drug effects, compliance, and initial doses of antihypertensive drugs recommended by the Joint National Committee vs the Physicians' Desk Reference.
BACKGROUND Compliance problems are common causes of the inadequate treatment of hypertension, with 16% to 50% of patients quitting treatment within 1 year. Dose-related adverse drug events (ADEs) frequently cause compliance problems, and many ADEs occur with the initial doses of antihypertensive drugs. Thus, it is an established tenet to initiate antihypertensive therapy at low doses to avoid A...
